Disclaimer: Semper Solutus provides medical documentation services and educational information. We do not prepare or submit claims or represent veterans before the VA. Federal contracting representations on this page describe Semper Solutus capabilities and registration posture only and do not constitute a commitment by any federal agency.
Semper Solutus is a Veteran-Owned medical documentation firm registered to do business with the U.S. Government. We support federal agencies and prime contractors with records-based independent medical opinions (IMOs), independent medical examinations (IMEs), federal physicals, clinical quality review, and medical records analysis. Our work product is authored by licensed Medical Doctors, structured to meet federal evidentiary standards, and produced under documented quality control and HIPAA-aligned controls.

What We Provide Under Federal Contract

Independent Medical Opinions (IMO)

Records-based written opinions authored by licensed MDs addressing causation, aggravation, secondary connection, or rebuttal of prior medical findings. Structured to the "at least as likely as not" evidentiary standard used in VA adjudication, federal workers' compensation under FECA, and federal court IMEs.

Independent Medical Examinations (IME)

Full IME services including in-person and records-based examinations for federal agencies, prime contractors processing federal claims, and government-aligned legal teams. Reports follow agency-specific reporting templates and timelines.

Federal Physicals

Pre-employment, fitness-for-duty, return-to-work, periodic, and separation physicals for federal employees and contractor personnel. Reports formatted to OPM, OSHA, DOT, and agency-specific medical standards.

C&P Exam Quality Review

Independent clinical review of completed Compensation & Pension examinations and Disability Benefits Questionnaires for clinical sufficiency, evidentiary adequacy, and consistency with the medical record. Used by federal oversight teams and prime contractor quality assurance programs.

Psychological Evaluations

Comprehensive psychological and neuropsychological evaluations authored by licensed mental health practitioners. DSM-5-TR-aligned and structured to federal evidentiary standards for PTSD, MST, TBI sequelae, and related conditions.

Medical Records Review & Analysis

Systematic review and clinical analysis of service treatment records, VA records, private treatment records, imaging, and lay statements. Output includes record indices, clinical timelines, and physician-level narrative analysis.

Clinical Subject Matter Expert Support

MD and licensed clinician SME support for federal contract performance, peer review panels, clinical policy development, training content review, and expert testimony preparation in administrative and federal court proceedings.

Surge Capacity & Subcontract Support

Available as a subcontractor to incumbent VA Medical Disability Examination (MDE) IDIQ holders and other federal prime contractors needing additional MD-authored records review or geographic exam capacity.

Why Federal Agencies and Primes Engage Semper Solutus

MD-Authored, Not Reviewed

Every clinical opinion is written by a licensed Medical Doctor. We do not use a paraprofessional drafting model with physician sign-off. The medical analysis, the rationale, and the conclusion are the work product of the credentialed physician whose name and signature appear on the document.

Records-Based Methodology

Our default methodology is a full records-based review. This produces opinions that are reproducible, auditable, and consistent with how federal adjudicators and Boards weigh probative value. The records drive the opinion, not self-reported history alone.

Veteran-Owned, Mission-Aligned

Semper Solutus is veteran-owned. Our team understands the federal evidentiary environment, the operational tempo of agency timelines, and the documentation standards required for federal acceptance because we have lived inside those systems.

Documented Quality Controls

Every work product goes through a documented clinical quality review prior to delivery. Our quality program is structured to support federal audit, FAR Part 46 quality requirements, and prime contractor QA flow-downs.

Scalable Geographic Coverage

Our records-based delivery model is location-independent. Records-only review produces nationwide capacity without onsite footprint, and we maintain a vetted network of credentialed examiners for in-person IME requirements.

Clinically Conservative, Legally Defensible

We do not produce favorable-conclusion documents on demand. Every opinion reflects the actual clinical reasoning of the physician based on the records. This is the standard federal adjudicators expect and the standard our work product is built to meet.

Federal Contracting Questions

Yes. Semper Solutus maintains an active registration in the System for Award Management (SAM.gov), the federal government's primary contractor registry. Our Unique Entity Identifier (UEI) and Commercial and Government Entity (CAGE) code are provided to verified contracting officers and prime contractor business development teams as part of our Capability Statement package. Email nexus@sempersolutus.com to request.

Both. Semper Solutus is positioned to prime opportunities sized within our SBA size standards under our primary NAICS codes and to participate as a subcontractor or teaming partner on larger vehicles such as the VA Medical Disability Examination (MDE) IDIQ and the DHA contracting portfolio. Teaming inquiries from primes seeking veteran-owned medical capacity are welcome.

Yes. PHI handling is core to our service offering. Semper Solutus maintains HIPAA-aligned administrative, technical, and physical safeguards consistent with 45 CFR Parts 160 and 164. All intake and delivery channels use encrypted transmission, and access is controlled on a minimum-necessary basis. Privacy Act considerations applicable to federal agency records are addressed in contract-specific Data Use Agreements and Business Associate Agreements as required.

Standard turnaround for records-based IMOs is contract-dependent and is negotiated against the volume, complexity, and reporting template required. Our internal target for standard-complexity records-based IMOs is within ten business days of complete records receipt. Surge configurations and expedited timelines are available where the contract or task order requires faster delivery. Specific service level commitments are documented in the contract or task order.

Both. Our default delivery model is records-based, which produces nationwide capacity without onsite footprint. For requirements that include in-person examinations, federal physicals, or hands-on clinical components, Semper Solutus engages a vetted network of credentialed examining clinicians and coordinates scheduling, intake, and reporting under our quality program.

We are not an MDE IDIQ prime. The four MDE primes (QTC Medical Services, OptumServe Health Services, Veterans Evaluation Services, and Loyal Source Government Services) hold the IDIQ awards. Semper Solutus is positioned to support these primes as a subcontract provider of MD-authored records-based opinion and quality review capacity, particularly for higher-complexity records that benefit from additional clinical depth or independent peer review prior to delivery to the VA.

All federal contracting correspondence should be directed to nexus@sempersolutus.com. Use the subject line format [Agency] - [Vehicle / Solicitation #] - [Inquiry Type] to route correctly. Capability Statement requests are returned within two federal business days. For active sources sought or RFI responses, include the SAM.gov notice number and response deadline.
